The Safe Steps COVID-19 Core Data Tracker is a simple, quick and easy-to-use digital tool for carers to provide daily updates on the health status of their residents.
The clinical data collected using the tracker provides an accurate and standardised assessment of acute and non-acute information, enabling proactive clinical response in a timely manner. Access to this data enables better planning and care of residents in this and future outbreaks.
Additionally, the tracker supports wider personalised healthcare needs of residents in care (RESTORE2TM).
Key features
The tracker facilitates proactive identification of residents with COVID-19 symptoms supporting outbreak management, resident assessment, interventions and care planning. The symptom tracker captures valuable data including:
- Insights into residents with delirium – something which is not routinely recognised in care homes currently – but is a presenting symptom of CV19 or other acute illness
- Profile of illness responses and trajectories for COVID-19 symptoms and delirium in a way that will support longer-term resident monitoring
- A real-time way of quickly finding all those residents with escalating needs – within homes and across a large system of homes – in a way previously not possible
The tracker is also a mechanism for assurance at a home and system level for delivery of the National NHS commissioning requirements to support the roll-out of the Enhanced Health in Care Homes Direct Enhanced Service (DES) and NHS standard contractual requirements for community services support to care home residents.
