In addition to improved PROMs, a randomised controlled trial demonstrates a 34% reduction in A&E attendances and a 24% reduction in non-elective care.

HN deploys artificial intelligence on the broadest possible range of clinical and operational data to predict and identify high-cost, high-need patients. Our team of healthcare professionals, onboard these patients to HN Clinical Coaching CARE – a high intensity clinical coaching and navigation intervention lasting around six months.
Supported by advanced patient identification models and digitally enabled patient-coaching, the programme supports patients to achieve rapid improvements in self-care and promotes healthy ageing. Furthermore, patients receive care coordination and education from the HN team.
This is a nurse-led service, delivered by telephone and video-call depending on the patient’s preference. Each patient receives support from a named health coach, ensuring a strong patient/coach relationship is built.
To support the NHS during the pandemic, we developed HN COVID-19 CARE – a service to proactively monitor, support and coach patients whose medical conditions put them at risk of becoming seriously ill with COVID-19. The service is delivered by healthcare professionals, who complete a holistic assessment of the referred patient. The patient is monitored, coached and supported by telephone and/or video before meeting the discharge criteria. The service is COVID-secure and provides assurance to patients and clinicians, without the need for a face-to-face interaction.
Due to the pressures faced by the NHS and the backlog of cases created by the lockdown, we are developing the predictive capacity of our algorithm, to find patients before they need healthcare. This service will utilise primary care data.
